Name Description

Terren Peizer

Mr. Terren S. Peizer is Chairman of the Board, Chief Executive Officer of Catasys, Inc. (formerly Hythiam, Inc.). He is the founder of Company and has served as Chief Executive Officer and Chairman of Board of Directors since inception in February 2003. Since September 2009, he has served as the Chairman of Crede Capital Group, LLC, his personal investment vehicle, and industry leader in investing in micro and small capitalization equities, having invested over $1 billion directly into portfolio companies. Mr. Peizer has been the largest beneficial shareholder, and has held various senior executive positions with several technology, biotech, and healthcare services companies. He has assisted companies by assembling management teams, boards of directors and scientific advisory boards, and formulating business, capital formation, and investor relations strategies for the companies. Mr. Peizer has a background in venture capital, investing, mergers and acquisitions, corporate finance, and previously held senior executive positions with the investment banking firms Goldman Sachs, First Boston and Drexel Burnham Lambert. He received his B.S.E. in Finance from The Wharton School of Finance and Commerce.

Richard Anderson

Mr. Richard A. Anderson is President, Chief Operating Officer of Catasys, Inc. (formerly Hythiam, Inc.). Mr. Anderson has served as a director since July 2003 and as a member of our management team since April 2005. He has been our President and Chief Operating Officer since July 2008, in this role he has been primarily responsible for the creation of our managed care OnTrak program. He has more than twenty years of experience in business development, strategic planning, operations, finance and management, with 15 years of that in the healthcare field. Prior to joining the Company, he held senior level financial and operational positions in healthcare and financial companies, and served as a director in PriceWaterhouseCoopers LLP’s business assurance and transaction support practices. He received a B.A. in Business Economics from University of California, Santa Barbara.

Christopher Shirley

Mr. Christopher Shirley serves as Chief Financial Officer of the Company. Mr. Shirley joins the Company with approximately 20 years of finance experience, including senior leadership roles at healthcare technology and big data companies. Most recently, Mr. Shirley served as the Chief Financial Officer of Sentient Science Corporation from September 2016 until February 2017. Previously, as CFO of GE Intelligent Platforms from March 2015 until September 2016, he led the finance function during a period of rapid expansion. Prior to joining GE Digital, from March 2014 until March 2015, Mr. Shirley was the Financial Integration Leader for GE Healthcare, where he led the financial integration and delivery of deal model expectations following its acquisition of API Healthcare. Before his role as Financial Integration Leader for GE Healthcare, Mr. Shirley served as Global Finance Manager of GE Healthcare beginning in June 2011.

Jeremiah Stone

Mr. Jeremiah Stone is Chief Technology Officer of the Company. Mr. Stone joins Catasys from GE, where he most recently served as Vice President of Applications for GE Digital. In this role, he led creation and delivery of the industry-leading application platform underlying predictive analytic-centered products and services across GE's nearly $50 billion services franchise. Prior to GE, Mr. Stone was Vice President for Sustainability and Natural Resource Industry Solutions at SAP. Mr. Stone's 20 years of industry experience have resulted in deep domain knowledge across platforms, applications and analytics.

Richard Berman

Sharon Gabrielson

Ms. Sharon Gabrielson is Director of the Company. She has dedicated over 35 years to the healthcare industry and has extensive industry experience in technology, finance, human resources, insurance, mergers and acquisitions, and business development. Ms. Gabrielson currently serves as Chair of the Mayo Clinic's Global Business Solutions Department where she is transforming an underperforming unit into the heart of Mayo's business diversification and profitability. She directs the domestic and international new business development opportunities for commercializing the Clinic's competencies to deliver service-based products to the B2B and B2C markets. Ms. Gabrielson has also served as Administrator of the Clinic's Office of Population Health (2014-15) and Vice Chair of Health System Administration (2010-15). In these roles, she was instrumental in developing a strategic plan for market penetration, scaling for growth and competing in a risk-based environment. In 1982, Ms. Gabrielson began her career as a registered nurse at Saint Mary's Hospital where she served as a staff nurse and a charge nurse until 1990.

Diane Seloff

Ms. Diane Seloff is Director of the Company. Ms. Diane Seloff possesses over 25 years of experience in the broader healthcare industry and has diverse experience in operations, human capital, finance, technology, strategic planning and business development in healthcare, financial services, and logistics. Since 2013, Ms. Seloff has served as Chief Operating Officer at Aspire Health where she was part of the founding leadership team. In the past five years, the Nashville-based startup has grown to become the nation's largest provider of home-based palliative care and was acquired by Anthem in June 2018. As COO, Ms. Seloff built all the corporate functions for the company in its first two years and is responsible for running and transforming Aspire Health's operations. She previously held senior executive roles in operations and development at other healthcare services and technology companies, including Vanderbilt University Medical Center. Ms. Seloff began her career as a management consultant—first at Arthur Andersen & Company and later at KPMG Peat Marwick.

Michael Sherman

Mr. Michael Sherman serves as Director of the company. Mr. Sherman is currently on leave as a Managing Director at Barclays Plc, where he worked from 2008 to 2017. Prior to this position, from 1998 to 2008 Mr. Sherman was a Managing Director at Lehman Brothers, Inc. Mr. Sherman has worked in finance for 30 years, having begun as a securities lawyer with Cleary, Gottlieb, Steen & Hamilton in New York and Hong Kong and having served as an investment banker at Salomon Brothers, Lehman Brothers and Barclays. Mr. Sherman was a Managing Director in Equity Capital Markets at both Lehman Brothers and Barclays, covering Healthcare companies, in addition to companies in other sectors. Mr. Sherman received a B.A. in History, magna cum laude, from the University of Pennsylvania. Mr. Sherman also received a J.D., cum laude, from Harvard Law School.

Edward Zecchini

Mr. Edward J. Zecchini is Director of the Company. Mr. Edward Zecchini has over 30 years of experience in the healthcare and information technology industries. Since 2014, Mr. Zecchini has served Chief Information Officer for Remedy Partners where he is responsible for building the technology and infrastructure necessary to support the implementation of bundled payment programs and other episode financing business lines. From 2010 to 2014, he served as Executive Vice President and Chief Technology Officer of Sandata Technologies, LLC, the leading provider of information technology solutions to the home healthcare industry. He also previously served as President and CEO of IT Analytics LLC, a technology consulting firm, and as Executive Vice President of Operations and Chief Information Officer of Touchstone Healthcare Partnership.

Richard Berman

Mr. Richard Berman is Independent Director of the Company, since February 01, 2014. Mr. Berman is currently the President and Chief Executive Officer of LICAS, a K-12, College and University, Health Care consulting firm. In addition, he currently serves as Chairman of the board of directors of Emblem Health's Quality of Care Committee and a member of its Audit Committee. Mr. Berman is also an Entrepreneur in Residence at GaTech’s ATDC and visiting professor at USF MUMA College of Business. He is an elected member of the Institute of Medicine of the National Academy of Sciences. Previously, he was a management consultant for McKinsey & Company, Executive Vice President of NYU Medical Center and Professor of Health Care Management at the NYU School of Medicine. He has also held various roles at Korn Ferry International, Howe-Lewis International, the New York Office of Health Systems Management, US Department of Health Education, and Welfare, and as the President of Manhattanville College. In 1995, Mr. Berman was selected by Manhattanville College to serve as its tenth President. Mr. Berman is credited with the turnaround of the College, where he served until 2009. In 2006, Mr. Berman was awarded a Fulbright Commission grant to travel to Uganda and provide strategic planning and leadership training to Kabale University. Mr. Berman has a Bachelor of Business Administration, an MBA and Master in Public Health.
